The type of personal information I collect
I am the data controller and data processor for any information you give to Eagleheart Reiki. I currently collect and process the following information:
- Your name, address, and contact details.
- Details of an emergency contact, and the name of your GP surgery.
- Your general health, medical history and pregnancy status.
- Your medicine, alcohol and drug consumption.
How I get the personal information and why I have it
The personal information I process is provided directly by you for one of the following reasons under the General Data Protection Regulation (GDPR).
I use the information that you have given me in order to give you the safest and most helpful reiki experience possible, fulfil my legal duties, and comply with all required policies and insurance requirements.

I also may collect personal information indirectly from the following sources in the following scenarios:
- If you become unwell or have an accident and I need to call your emergency contact, they may tell me extra information about your health. I will not add this extra information into your notes without your consent.
I also have a legal duty of care to inform your GP if you tell me that you may be a danger to yourself or to other people.
How I store your personal information
During your first visit, you will complete a questionnaire and sign a consent form. These forms will be stored in a locked filing cabinet. I will then make a digital copy of this information, and securely dispose of the paper copy.
After each visit I will make notes about the session and include information you give me about what you experienced. These notes will be made on paper and kept in a locked filing cabinet. I will then make a digital copy of this information, and securely dispose of the paper copy.
I will delete all digital copies of your information 10 years after your last reiki session. If you withdraw your consent for me to hold your data, I will delete all digital copies and destroy all paper information within 14 days.
Your data protection rights
Under data protection law, you always have rights over your personal information, including:
Right of access – You have the right to ask for a copy of all information I hold about you.
Right to rectification – You have the right to ask me to rectify personal information you think is inaccurate. You also have the right to ask me to complete information you think is incomplete.
Right to data portability – You have the right to ask that I transfer the personal information you gave me to another organisation, or to you, in certain circumstances.
Right to erasure – You have the right to ask me to erase all of your personal information (except information that I legally need to store). This will also remove you as a client.
You will never be required to pay any charge for exercising your rights. If you make a request, I have one month to respond to it.
